Paramedic students have the fear god put into them to NOT overdose on pain meds, hence chronic under dosing, encouraged by narrow guidelines. AV and AAV took their revised Paed Pain relief CPG’s tonthe RCH Melb. The recommendation was that min doses should be doubled and max doses increased 10 fold (ie severe burns)

This is an alarming statistic. CPGs are there for a reason, derived from current medical evidence and governed by those who prefer conservative dosage regimes. If the patient is in significant pain, it’s our duty to provide relief no matter the age.
